OGG134A		 Designation of Agent (for corporations and limited liability companies only)
The assigned agent shall serve as the contact person for all Division communications. Must reside within the state and provide a street address.

OGG30A 		Report of Property/Well Transfer or Acquisition
File within 30 days of the sale, assignment, transfer, conveyance, or exchange of well ownership. Include company letters by both parties stating the transfer and acquisition.

 OGG160A		 Individual Geothermal Resources Well Indemnity Bond
File in the amount of $25,000 to cover well operations on an individual well. You must use an authorized surety company. This amount does not apply to low-temperature wells. See PRC 3725.5

OGG160B		 Blanket Geothermal Resources Well Indemnity Bond
File in the amount of $100,000 to cover all well operations (in lieu of individual bonds for each well). You must use an authorized surety company.
